A Proposal for Funding Cover Sheet containing information: Name of
Organization, address, Executive Director; Contact person other than
Executive Director; Telephone, Email, Fax and Website; Amount
requested; Project Budget if applicable; Total annual organization’s
budget, fiscal year ending; United Way support?; Type of request
(operating, project, capital, other).

A Letter setting forth what you need and why, what it will accomplish
if funded, and who it will serve.

A budget for the project and/or your organization and financials.

GRANT PROCESS..........
The Board of Directors meets in May and November. Applications for funding
will be accepted prior to April 15 and October 15. A full proposal is preferred.
Associated Grant Makers of Massachusetts Common Proposal Form and
Report are acceptable (
agmconnect.org). Detailed application guidelines will
also be supplied upon request. A grant request should supply the following
